The incidence of end-stage renal disease (ESRD) is 100-fold higher in subjects older than age 65 compared with people younger than 20 years. ESRD is usually accompanied by a complex pattern of alterations in lipoprotein metabolism and structure. Mechanism that may link dyslipidemia to the progression of glomerular disease are similar to the pathophysiology of atherosclerosis. Because of its direct contact with the circulating blood, the endothelium is particularly expose to the modulatory effect of altered lipoprotein particles. Data reported in literature show that the treatment of uremic dyslipidemia may be essential for the prevention or the delay of cardiovascular complications in ESRD. This review characterizes dyslipidemia in patients with ESRD and evaluates the effects of treatment on renal disease progression.

Objectives and methods: life expectancy for subjects with Down syndrome (DS) has increased in the last decades. The aim of this study was to investigate the socioenvironmental factors which can affect the quality of life of 173 DS subjects (age range: 18-65 years) attending the ViviDown Association in Milan.

Results: the younger subjects (<= 34 years old) had a higher level of scholarship and visited more frequently the Social Education Centers.

Conclusions: we conclude that socioenvironmental factors may be responsible for improving clinical aspects and quality of life of DS subjects.

Objectives: aim of our study was to compare tariffs and costs for older patients admitted to long-term care in hospital, nursing home and home care in region
of Marche.

Materials and methods: we studied health date from:
a) patient´s dimission discharge note of nursing home in the 2002 and of the long-term care in hospital,
b) accountancy date-base of the 2002, c)date-base of the patients admitted to Fabriano´s home care .

Results: patients admitted in three different health levels, long-term in hospital, nursing home, home care, attributed to 25 Major Diagnostic Categories (MDC) to
comparison direct costs with reimbursement from National Health Service. In this way it was possible to obtain daily costs for each care options.

Conclusions: comparison regardig costs in the different care options show that it is more convenient to assist older people in their home. A detailed day per day
monitoring of hospital and nursing home, would allow discharge patients with low health care demand to home care, in this way it is possible saving costs for
the community.
